How to enroll

1. 1 Print the Self-Exclusion Request Form from the WV Lottery site, have it and a copy of your government-issued ID notarized, and mail both to the West Virginia Lottery, 900 Pennsylvania Avenue, Charleston, WV 25302

What to watch out for

There is no menu of term lengths: exclusion is indefinite, with a minimum of one year before you may apply for removal using the separate Self-Exclusion Removal Application, so removal is possible but not automatic.
